-If you are not able to keep your scheduled appointment time, please give the office a 24 hour cancellation notice.
-We will complete FMLA forms. Please contact the office for specific instructions.
-Prescription refills take 3-5 days to sign off on, so please let your pharmacy aware of this 1 week before you run out of medication.
-Referrals are the patient's responsibility and if your insurance does require one, you must contact your primary doctor. If you do need a referral and are unable to get one, we will have to reschedule the appointment.
